The pro’s and con’s of saline vs. silicone are often vigorously debated. Dr. Amy Bandy fells that the most natural look and feel comes with the silicone gel implants, especially in women who have minimal to no breast tissue. In women who have larger breasts, the difference between the two implants becomes less and less.

When considering breast augmentation, women should be informed that the implants are not to be considered a lifetime device. The implants come with a lifetime warranty from the manufacturer, however it is recommended that if you have silicone gel implants, that you have a baseline MRI after 3 years, and implant exchange every 10 years. There is a very low percentage risk at 10 years that there is a defect in the implants, and that makes it easier to have a re-operation that is minimally invasive. With saline implants, there is no specific recommendations, and you will certainly know if there is a leak, since the implant will deflate.

Dr. Amy Bandy allows the patient to choose their incision site, after fully discussing their options and the pro’s and con’s of each. The incisions can be either under the arm also known as axillary incision, around the areola, or under the breast in the natural fold, called the infra-mammary incision.
Once the incision has been made, the soft tissues are lifted until the border of the pectoralis muscle is encountered. For women who choose to have their breast implants placed behind the muscle, the muscle is then elevated and a pocket is created under the muscle. In select patients who choose to have their implants above the muscle, the breast and fatty tissues are elevated over the muscle to create the pocket.
Dr. Amy Bandy uses a “no touch” technique to place the implants, as this has been shown to significantly lower the risk of infection and capsular contracture. Patients have the option for the Keller Funnel, which is a no-touch method of implant placement.
Once the implants are in their proper position, the incision is then closed with an absorbable suture below the skin level so that the sutures do not need to be removed.
Patients should rest for the first few days after surgery. Most activities and exercise can be resumed as soon as 2 weeks after surgery if they do not require upper body strength.
